Student Physical Dates
All student-athletes must have a valid physical on file to participate in sports. A photo of the physical (front and back) must be uploaded to FinalForms (add hyperlink) and must have a doctor's stamp to participate in practices and competitions. Below are the dates physicals will be offered by the district doctor:

Physicals expire 12 months after the date of health exam.

No athlete will be permitted to play or tryout with an expired physical.

No athlete will be permitted to play or tryout without parent/guardian permission.
