Central Registration
REGISTRATION REQUIREMENTS
While the office is open throughout the summer months, parents are strongly encouraged to register as soon as possible to avoid long lines and waiting time.

At the time of registration, parents, legal guardians, and social workers, must bring the following:
• Parent's Photo Identification
• Marriage Certificate (if applicable)
• Child’s original birth certificate. (out of country birth certificates must be translated in English)
• Proof of guardianship from court or foster care letter with authorization to register child and a completed DSS2999 form.
• Proof of residency (see below).
• Child’s up-to-date immunization records (proof of immunization for Polio, DPT, Rubella, Measles, Mumps, HIB, Hepatitis B, Varicella and Health Certificate (physical) and Dental Certificate must be stamped and signed by health care provider).
• Child’s Individual Educational Plan (IEP) if applicable.
• Report Cards from previous school.
All documents must be presented at the time of registration or the registration will not be accepted.
ACCEPTABLE PROOFS OF RESIDENCY
The following items are acceptable proofs of residency:
• Original Deed/Mortgage Statement or current lease (all handwritten leases must be notarized).
• If a parent is registering a child, and the child and parent are living with a relative who is legal resident of Hempstead, a District Affidavit must be signed by both parties and notarized. An original deed or lease of the owner, or the one leasing, must also be provided.
****IN ADDITION**** - Any two (2) of the most recent (within 30 days)
• Hempstead Water Bill
• School Tax Bill
• LIPA Bill/Cable Bill
• Bank or Credit Card Statement
• Drivers License
• Social Security Correspondence
• D.S.S. Correspondence
• Telephone Bill
These items should specify the name of the parent or guardian and the residence address.
All persons are subject to Hempstead School District home visits and other methods for further verification.
